Cognition Chronicle is a prescription mobile application (App) that is a Computerized Cognitive Assessment Aid, intended to be used as a self-administered tool for patients who are under routine medical care, ages 18 to 89 years, to assess processing speed, spatial memory, and verbal memory as a means of assessing cognitive capabilities. Distribution through the App Store is limited to health care providers (HCPs) who have been given access to install the App on an iPad; similarly, access to the Chronicle Web portal and App will be restricted by user account creation.
Cognition Chronicle is intended to be used in a health care setting where the HCP initiates the assessment, followed by the patient completing the tests in the order predefined in the assessment. Upon completion of the assessment, the mobile application outputs the patient’s raw score from each test to the API Server. On the API Server, summary scores for each test are calculated, including the option to calculate demographically adjusted scores. Raw and demographically adjusted scores allow an HCP familiar with these tests to independently review the results, both cross-sectionally and longitudinally, and use them as they wish in conjunction with other standard medical assessments or tools to evaluate the patient’s condition. HCPs should not rely primarily on Cognition Chronicle to make a clinical diagnosis or treatment decision for the patient.
